Our clinical nurse consultancy service will aim to provide quality support and care for clients with mental health challenges. Our service will help participants of NDIS and their carers to develop a wellness plan based on an individualised holistic approach. There are 3 phases to the implementation of the plan: relapse support, recovery and maintenance. Each phase is based on the needs of the client at the time. The aim is to assist clients to become as independent as possible through their mental health journey.
